Diamond Status Facilities
We are pleased to announce that 60 facilities in our Network are actively participating in the 5-Diamond program. That's 36.8% of all eligible Network 1 dialysis providers (numbers do not include participating community partners).
Facilities that are enrolled in the program are listed below, including their diamond status*. Congratulations on your accomplishment and thank you for your devotion to patient safety!
5-Diamond status achieved between:
- March 1, 2009 and March 1, 2010 is good through March 1, 2011.
- March 1, 2010 and March 1, 2011 is good through March 1, 2012.
- March 1, 2011 and March 1, 2012 is good through March 1, 2013.
5-Diamond facilities may renew diamond status for an additional year by completing two (2) additional modules. Facilities in BOLD text have maintained their 5-Diamond status for more than 1 year.
